Chronic disease management plans

Our GPs and nurses are committed to ongoing care and management of both acute and chronic medical and related conditions.
We offer GPMP(care plans) and TCA (referral to allied health) plans all in accordance with the Medicare schedules.
We also offer domestic medicare management reviews (DMMR). Ask your doctor about this. A pharmacist can come out to you to review your medications at home, in consultation with your GP.
